SCCC-Watts Intake Request Form

This intake request form is specifically for our SCCC-Watts location. If you are interested in our Mid-City location (or you don’t have a preference), please use this intake form.

Please fill in the Intake Request Form to request an intake session with an SCCC counselor. All intakes are currently taking place via video conference.

If you think you may have a medical or mental health emergency, please call 911.

After completing an intake, you will be placed on our waitlist to be assigned a counselor for ongoing therapy. The current waitlist is approximately 2-3 months for individual counseling and 1-3 months for couples, kids/teens and family counseling.
