Intake Request Form

Please complete the Intake Request Form to request an intake session with an SCCC counselor. All intake sessions are currently conducted via video conference.

If you are experiencing a medical or mental health emergency, please call 911.

After your intake session, you will be placed on our waitlist to be matched with a counselor for ongoing therapy. Your first four sessions of ongoing therapy are provided at no cost. After that, your fee will be determined based on a sliding scale.
